Forus is partnering with the @AmerGastroAssn to improve medication access for gastroenterology patients and ease the burden on GI providers and practices. Together with AGA, Forus is working to bring better infrastructure and visibility to the access challenges GI practices and patients face every day, connecting the dots between evidence-based clinical guidelines and everyday practice. But how do we measure whether it's right? A gold standard tells us when the judge approves correct answers. It can't tell us when it misses wrong ones, the more dangerous failure. We take correct answers and perturb them into plausible but wrong versions, then watch whether the judge notices. Critically, perturbed answers have to be wrong for this specific patient, not just “wrong-looking.” We feed the model the patient's diagnoses, medications, and therapy history to keep the fake answers grounded in the real chart.

Most evals can tell you when an LLM judge is right, but not when it's silently wrong. At Forus, our LLM judge reads prior authorization forms and decides whether a patient's medical record supports them. A missed error delays someone's medication. To catch those failures, we built a way to generate realistic wrong answers and test the judge against them. Here's how 🧵
