Service failure is everywhere. Here's an example of the #nhs e-Referral service...
I was referred in March for a hospital appointment by my GP (local doctor surgery).
I then received a letter in April saying 'you need to book an appointment' using the e-referral service…

@GaryMcAllister The other factor in this, is where RPA is used, reduces the demand/pressure upon industry suppliers (including @NHSDigital) to implement an API.
