I actually don't find urinary indices like FeNa or FeUrea very helpful. Practices may vary among nephrologists but I find that most also don't put much stock in them. H&P plus urine Na to see if sodium avidity can be mounted are usually sufficient

@luke_hedrick Only thing I might add is that Uosm is not a perfect proxy for ADH. Can be high because the kidneys are dumping other solutes out, e.g. urea when recovering from an AKI. Can estimate the electrolyte-free water clearance in these scenarios

What I’ll miss most about residency is the amount of time we have to teach and learn together, so receiving the #LowellMcGee teaching award from the interns and my co-residents meant so much 🙌🏼🙌🏼

ACE inhibitors and ARBs are not “nephrotoxins.”
They improve renal blood flow.
But they reduce GFR, and impair potassium elimination.
Which is why we might hold them in [severe and/or worsening] AKI.
