Anthropic won't admit it until enough people complain, but Claude is broken right now. Opus 4.6 has been lobotomized. It burns through tokens accomplishing nothing. Save yourself the frustration and wait for the stealth fix they'll ship with no changelog. Wait it out.

Spend 1 hour with Opus 4.6 (high) in Claude Code. My god it was so dumb today, even the most basic stuff. Back and forth, even had to share function names in the prompts. Opened Codex, done in 1 prompt. How can Opus quality varies so much? It’s unreliable.
