@CMichaelGibson Good data showing Plavix is a great drug. However, ~30% Asians and ~15% Caucasians don’t activate plavix properly (CYP2C19 genetic issue). In elderly frail patients with frequent falls, a big group, aspirin has a role since it is short acting & easy to reverse

@cardiac_md ACC/AHA vs USPSTF: ACC/AHA treats CAC as a risk tool, needing only predictive accuracy + reclassification. USPSTF requires outcome trials (same bar as a therapy), which CAC lacks.
CAC moved from IIb→I because it became built into the PREVENT risk calculator & decision pathway

2013 ACC/AHA: Class IIb rec for CAC
2018 USPSTF: insufficient evidence for CAC
2018 ACC/AHA: Class IIa rec for CAC
2019 JAMA review: insufficient evidence for CAC commonly ordered for executive physicals.
2023 ACC/AHA class IIa rec for cac
- With no new clinical trial or any compelling evidentiary change,
2026 ACC/AHA: FIVE Class I rec for CAC.

Don't understand the dichotomy on this issue in the cholesterol management guidelines.
1. CAC>100: Class 1 rec intensive LLT management, LDL target <70 (AGREE)
2. Moderate to severe incidental CAC on non gated , where almost exclusively you will have close to create than >100 on gated: Class II for same LDL goals <70 (DISAGREE WITH CLASS REC)
Any thoughts on this?

We used to drive to the airport at night. I was in my twenties—exhausted from med school classes / library and hospital rounds, but still full of hope. The airport was close, and it felt like the edge of the world. We’d sit for hours, watching planes take off, engines roaring into the dark, and I’d think, One day, I’ll be on one of those. I’ll fly away. I’ll make something of all this. One day I took my first flight in life. It was Ahmedabad- London- Philadelphia!
All of those nights watching those planes- world was open and waiting for me..
And today, hearing the news—
The crash.
Along with all 242 passengers - The unofficial reports of close to 75 medical students- Gone. Their dreams- gone.
It’s such a sad day.
Each of them had stories. Books they hadn’t finished. Futures they were working for. Maybe they watched planes take off too, thinking the same thing I did.
This feels so personal.
So live. Don’t wait. Take the trip. Quit the job. Say what you need to say. Stop postponing joy.
Tomorrow isn’t promised.
None of this is.
All we really have is right now and today.

Heartbroken to hear this news! The plane crashed into my Med school Hostel and in addition 242 on board- many medical students and healthcare workers are affected. Some dead. Life is so cruel! Prayers for those affected victims and their families..
