@jmlogue Throwing to a wrong base accurately is a mental mistake and not an error, but a mental mistake like thinking there are three outs and flipping the ball in the standing is a throwing error. I’ve always found it weird but it is what it is.

This is straight from the OS Manual (not in the rule book) “Misjudgments While it may sometimes feel unfair to the pitcher, if a fielder misjudges a ball to the point that he is never able to get in position to make a play no error should be charged.” MLB and some of the top official scorers put the manual together. It’s not done in a vacuum.
