Morning all… any of my mental health older adult colleagues out there who work in a community OA home treatment team who would be willing to give me 30 mins to discuss. Doing some service redesign and interested in models of delivery out there. Thanks in advance

@deliriumkrish @rcpsychOldAge Had the pleasure of knowing and spending valuable time with him. Great man, stood up for his beliefs but even more-so those of his patients ! He turned up to a CHC DST meeting once for one of his memory clinic patients. Nobody knew what had hit them when it was all over 😢

Saddened to hear the news of Prof Dave Jolley who passed away on 17 May He was one of the forefathers of Old Age Psychiatry . I was fortunate to have had brief encounters with him. May his soul rest in peace. @rcpsychOldAge

@drgdubya @wendyburn @LeedsGp @LeedsLMC @LeedsHospitals Morning…. Would it be possible to contact you re this piece of work. We are looking at discharge letters/summaries as part of our in patients transformation work. Looking at what others are doing well would be really helpful. Thanks 😁

@wendyburn Hi Wendy , @LeedsGp and @LeedsLMC did some great work with @LeedsHospitals co- designing their discharge summary. We have an agreed set of principles with ideals around content and structure that work to help everyone .It would be great to share what we did . Can you DM me ?

GPs we are looking at our discharge summaries. I know what you don’t want (“GP to kindly” will be banned) but what would be helpful? Often long admissions with complicated treatment.

@DrKateLovett We do a triage call for info gathering then F2F in clinic. If diagnosis given and treatment (or not) seen again for post diagnostic support and then transferred to our review service. We have a non discharge pathway in Staffordshire so keep all!!!

@Trisha_the_doc @betterageing @NHSEngland My biggest moan on my ward! My dementia patients may have an off day and that’s it dipstick and I’m told its lit up like Christmas and then spend days trying to get a sample or call duty doc who will start antibiotics. considering having badges made “ dipstick- just say no!

Flowchart from @NHSEngland. “Do not perform urine dipsticks: Dipsticks become more unreliable with increasing age over 65 years. By 80 years half of older adults in care, and most with a urinary catheter, will have bacteria present in the bladder/urine without an infection.”
