Published today, Screening, Diagnosis, and Management of Pediatric Hypertriglyceridemia A Scientific Statement From the American Heart Association. Triglycerides enter the circulation by ingestion [BE1.1](exogenous pathway) and endogenous production. Triglycerides within chylomicrons and VLDL are hydrolyzed by endothelium-bound LPL, which releases free fatty acids (FFAs) and leaves remnant particles. LPL function is enhanced by apo CII, apo AV, intracellular chaperone lipase maturation factor 1, and GPIHBP1 (glycosylphosphatidylinositol anchored HDL-binding protein 1). LPL function is downregulated by apo CIII and ANGPTL3 and ANGPTL4 (angiopoietin-like proteins 3 and 4).21 📷 Triglyceride metabolism

👉The Long-Term Efficacy and Safety of Evinacumab in Patients With Homozygous Familial Hypercholesterolemia (HoFH) ☝️ PERSPECTIVES COMPETENCY IN MEDICAL KNOWLEDGE: Patients with HoFH are difficult to treat, with standard-of-care LLTs providing limited LDL-C-lowering efficacy, especially in patients with absent or severely impaired LDLR activity. In this open-label extension of the phase 3 ELIPSE HoFH study, evinacumab, an ANGPTL3 inhibitor, demonstrated sustained LDL-C reduction in patients with HoFH regardless of LDLR function and irrespective of background LLTs. TRANSLATIONAL OUTLOOK: Although randomized clinical trials are the gold standard for determining the efficacy and safety of a treatment, the strict application of inclusion and exclusion criteria and monitoring of participants means that trial populations and observed treatment effects are often not representative of patient populations in a real-world clinical setting. Thus, real-world evidence studies are needed to determine whether the observed LDL-C-lowering effect with evinacumab in the clinical trial setting is replicated in real-world patients with HoFH and whether long-term therapy with evinacumab in HoFH patients will reduce cardiovascular events and prolong their lives. 🔓Open Access jacc.org/doi/epdf/10.10…
