Appreciative of the @WhiteHouse goal to ensure individuals in rural America have access to high quality, affordable #healthcare and would urge @CMSGov to increase opportunities for ALL #primarycare physicians to participate in alternative payment models.

The Community Health Access and Rural Transformation (CHART) model looks like it may help some rural providers participate in ACOs. #Primarycare is the lynchpin of all successful APMs so CHART MUST support physician-led models.
