@KelbermanNFL So if Payton just steamrolls him and continues as play caller, we’ll continue to see these BS wide receiver screens thrown behind the line of scrimmage going no where every 3rd play, never using the middle of the field or a TE. Awesome. Not sure it matters who has the OC title.

Agreed regarding "argument from authority." However, there is an opposite phenomenon which is also gross. Maybe grosser. Newly-minted surgeons with fancy pedigrees are often assumed to be incompetent by the local, experienced surgeons who are waiting with bated breath for a mistake or inadequacy that supports their confirmation bias that the surgeon sucks and the fancy pedigree is worthless. They bask in the irony, and share it widely among friends/consultants/OR staff, who then develop the same confirmation bias. In my experience, the local aces often have a hyperfixation on technical skills, and bash the new surgeon's "hands" while greatly underappreciating the importance of judgment, or what that surgeon could do in the long-term to improve patient care. Speed is also a major topic of discussion. "It took Dr ____ 5 hours to do this surgery, but Dr LocalBad@$$ can do it in 45 minutes!!!" Meanwhile, these new surgeons are simply new, slow, and uncertain like many fresh grads, and they need support/mentorship rather than active or passive sabotage. The new surgeon eventually sees how unsupportive the environment is, and leaves for another job. The senior surgeons then conclude that "Dr X couldn't make it five minutes in this town" without recognizing that they were a big reason why. I was admittedly lucky as a new grad since I was decently fast and had good social skills, but I watched many people suffer different fates. Surgeons reading this may very well have been on the delivering or receiving end but not fully understood what was happening.

@DrBruggeman This trend will continue to drive dissatisfaction with our profession which will inevitably be born out in our care whether we realize it or not. Surgeons need to unionize. We underestimate the power we have and continue to just put our heads down and complain while we grind on.

We have some final numbers for the impact on Orthopedics as it relates to the final rule for the physician fee schedule. There is no other way to spin it other than the numbers are bleak for independent physicians. While employed physicians will see a 2.5% reduction across the board, independent (non-vertically-integrated) physicians will feel the full brunt of the CMS decisions. These policies have the opposite effect of what was intended by CMS. When CMS proposed their rules, they stated that they were overpaying for facility fees and trying to account for consolidation. As a result of the changes, though, independent physicians like myself are being encouraged to go to hospital systems and insurers to ask for RVU-based contracts which are largely insulated from the major changes being proposed. Scoreboard for physicians: Employed (RVU-based) orthopedic surgeons - 2.5% decline Independent orthopedic surgeons - 2.3% to 11.6% decline @CMSGov needs to rethink their policy here. It has the effect of consolidating the market and increasing costs while simultaneously reducing quality, which was the opposite of their stated goals. Lack of inflationary updates and concerning policy changes like efficiency updates and facility/non-facility updates are leading to the substantially increased premiums that every person in the US is feeling.

@DrBruggeman This will be true for all subspecialty surgeons sadly. It seems many of us got into medicine at just the wrong time and physicians will continue to get bulldozed, with ever-increasing expectations for documentation perpetuating workaholism and burnout only to earn less pay.
