@kcbanaesthesia @dasairway @OAAinfo @IJOA_Journal What about when it becomes outdated this coming November at WAMM? 👀
English

How many definitions of difficult intubation in obstetrics are you aware of in the last 15 years ? authors.elsevier.com/a/1l4hq_,2Bc4y…
Time to adopt DAS definition universally @dasairway @OAAinfo @IJOA_Journal @VirtueOfNothing

@parthaskar I thought legal migration brings in money, skills and supports services. The social care despatately needs support and needs to be made attractive so that local people fill the vacancies. The need is to cut illegal migration and a strategy around it - but the govt has lost it.
English

“We don’t want foreigners and immigrants doing jobs”
Will you do them then
“That’s irrelevant. We just don’t want them”
#Brexit wasn’t enough
The need for self harm is greater now
Amazing

@amit_pawa @RegionalAnaesUK @nathaslam @ajrmacfarlane The RA-UK powerhouse in one pic. Hope the entire RA-UK team keeps inspiring us to do better for our patients and trainees.

This picture was taken just before #RAUK25 kicked-off
It features @RegionalAnaesUK :

Past Past President- me!
I have got to say, this was such an incredible meeting packed with knowledge, social interaction & entertainment! 👌🏾

So are you being dismissive of papers claiming high rates of TNS?
No from me as safer other options available - first do no harm principle applies. Just my thoughts though experts may have their own views.
English

But maybe things have changed?? We recently looked at a series of over 1000 lidocaine spinal anesthetics at our institution and found a TNS incidence of....0.2%. Wait, how? What has changed?? @saraamaralMD @AmandaKumarMD @wmbullockMDPhD @DrJeniMD @nelkassabany @ajrmacfarlane @deepak_neuro

English

Would you do a lidocaine spinal in 2025? When I talk to other anesthesiologists, the answer is not clear cut...🤔 #LidoSpinal #TNS @EMARIANOMD @amit_pawa @ASRA_Society @RAPMOnline @Assoc_Anaes @GongGasGirl @Ron_George @AmyPearsonMD @Wilkinsonjonny @ESRA_Society

@amit_pawa @rosie_hogg @elboghdadly Used in only 19% of patients with class 3 obesity.
RA-UK need to push for better training on Pre- puncture US.
Paper here - obstetanesthesia.com/article/S0959-…

English

Now @rosie_hogg takes on the “Truth” behind the papers for ultrasound guidance for spinals and says we cannot be serious that Ultrasound should be standard of care for Spinals!
#RAUK25
Don’t believe everything you see!
Unlike what @elboghdadly says, Ultrasound does take time

@ShaunLintern Hope trainees who rotate between the trusts can do the same. How many days a year are wasted on induction for quite a number of similar things taught in different trusts!!!

@DrHWazir We dont have a bizzare situation but the politicians and health leaders in the NHS have created this situation over the last 15 years. The Royal Colleges have watched this happen instead of unanimously coming together and taking the politicians, GMC and NHS England to task.
English

We have a bizarre situation in Britain where the public have to wait years for their operations, yet NHS doctors are out of jobs.
20,000 doctors will potentially have no training job this August. These jobs are being restricted by the government, who have taken no concrete action to address the jobs crisis. Meanwhile doctors are left with no choice but to change careers or move overseas in order to make ends meet.
Unemployed doctors and rising waiting lists. 21st century Britain.

English

@WightlinkTravel Its a 6-8 hour delay now.
Would it be unfair to expect some compensation Wightlink ? Why do customers are always at the receiving end? What / Where are our rights ?
English

I’ve won the #Anesthesia #History Association essay competition!
Rivers of Life: a brief history of circulation in life support. Explores 3 “streams” of keeping blood circulating, challenges western-centric origin of CPR, and raises some esoteric questions about survival.
English

@clistenes_cc Agree. We may do a sub-cohort analysis.
The last meta-analysis on obesity and raised BMi for neuraxial procedures published in Sci Reports also notes low number of pts with Bmi > 40 in studies.
English

@kcbanaesthesia Arguably an underuse of USG, especially given the population. Would you agree?
‘more needs to be done from the ground up’.
English

Should anaesthetists be using peri-operative ultrasound for neuraxial procedures?
Ultrasound is commonly used for venous access and peripheral nerve blocks.
Why not spinals and epidurals?

@clistenes_cc Thanks.
Prepuncture neuraxial US utilisation was < 20%.

@kcbanaesthesia @YavorRM @Anaes_Journal @elboghdadly Hi Kailash, we didn’t collect this information, so I’d need to check. However, we included 18 trials evaluating patients with obesity. Notably, even those without factors indicating difficulty—along with obese patients—showed improved efficacy with USG.
English

@amit_pawa @LloydTurbitt Never rated it that high and stopped doing it for caesarean birth performed under a GA.
English

Oooh -
Check this @LloydTurbitt - You said the “TAP block was dead” -
Maybe it is!

Anesthesiology News@anesthesianews
A trial has found that TAP blocks offer little pain relief following abdominal surgery and do not reduce opioid use. The popular method, thought simple and safe by many, needs reconsidering according to the researchers. anesthesiologynews.com/a/MCkBAA/t
English

@clistenes_cc @YavorRM @Anaes_Journal @elboghdadly Please check how many trials report with individuals > 40 kg/m2 and please come back to me.
