@SaltyEMNY Husband & I have been together 14 years; dated 10, married 4, expecting our 1st child. When we shared the news, a reaction we got “welcome to the years where you will talk about nothing but divorce.” Like seriously?! Every couple has issues, raising a child is hard, but come on?!

Hey can we stop telling young couples that they are going to hate each other in X number of years or that they're only happy because they haven't been together long enough yet? It's tacky and serves literally no purpose. Don't project your failed relationship onto me.

@MrsPA_C ACOG still recommends it. My community hospital’s practice is to check Rh status/give Rhogam when indicated. This is also how I was trained in residency, but some institutions are changing to the more recent consensus statement.

@MrsPA_C I recently had a convo about this w/ my ED pharmacist & OBGYN at my community hospital; the WHO & Society of Family Planning released a consensus statement in 2022: for both medical & surgical abortion < 12 weeks, recommend against anti-D immunoglobulin administration

@reverendofdoubt @CORD_EM This is a benefit to IMGs. But the problem still remains access to an EM rotation. At SGU, we we had an excel spreadsheet with elective rotation sites, but had to contact each one individually to set up a rotation. Not sure what the process is at other international schools.

@reverendofdoubt @CORD_EM Many international medical schools are not enrolled as a VSLO institution so IMGs are not able to apply for rotations through VSLO. With that being said, I was able to easily set up 3 EM rotations (2017), but that’s because my school (SGU) had numerous affiliations.

Important thread below w/ great comments. Is the VSLO system and need for SLOEs from EM rotations inherently IMG unfriendly and makes EM an incredibly difficult pathway for IMGs to pursue in U.S.? Something maybe @CORD_EM #CORD2023 can address?

@reverendofdoubt @CORD_EM Before the pandemic started, there was only 1 type of SLOE - one from an EM rotation with a residency program. Today, there is the eSLOE (from residency program), SLOE from non residency EM rotation, SLOE from EM sub-specialty rotation, oSLOE.

@reverendofdoubt @CORD_EM I’m going to use IL as an example b/c that’s where I’m from: there are 12 residency programs for EM; when I was setting up EM rotations, none of these programs accepted rotators outside of VSLO. I wasn’t able to rotate in IL, but I was lucky to still match at Rush.

@PhlawlessPharm EMS: pt coming in suspected overdose, not “responsive” (not talking)/acting bizarre; RR 15, Sp02 94%, pupils 3+ reactive. Already received 12mg narcan with no response prior to arrival, so we gave 2mg more.

@DrGRuralMD Had a pt sent to the ED - getting up from toilet to grab walker, fell on buttocks. Did not hit head, not elderly, reliable patient who clearly stated/ knew they did not hit their head, no LOC. But on eliquis, and rehab sent her to ED to get stat ct head?

@MaxJordan_N I’m an emergency doc and in the past 6 months, I have had a patient w/ a saddle PE, patient w/ 100 % RCA occlusion and a patient w/ a stroke. All of them late 20s-early 30s. None of them were post COVID. I just turned 32 today, and have been thinking a lot about these patients.

@ShariDunawayMD I was a told that I had to either take my nose piercing out or cover it with a bandaid (even after I explained it was a cultural thing) 😑😑 as if a bandaid on my nose wouldn’t be more distracting than a 1.5mm stud 🤷🏽‍♀️🤷🏽‍♀️
