Pakistan wants a strong healthcare system… but invests weakly in health. Pakistan’s health spending is around 2.9% of GDP. World average is around 6.7% of GDP. Pakistan’s health spending per person is around $39/year. World average is over $1,300/year. And then we expect doctors to work endless hours, face violence, manage overcrowded wards, handle emergencies, and stay motivated on low pay. Not realistic. Not sustainable. Not fair to doctors. Not safe for patients. A health system cannot run on slogans. It runs on staff, funding, protection, equipment and fair pay. When doctors leave, it is not only migration. Why Pakistani Doctors Are Leaving Pakistan? Every year, thousands of trained Pakistani doctors leave the country not out of lack of patriotism, but due to limited financial and professional opportunities at home. A young doctor in Pakistan earns around Rs. 65,000–70,000/month, while early-career doctors abroad earn several times more: 🔹 Pakistan: Rs. 65,000–70,000/month 🔹 UAE: ~Rs. 494,000/month 🔹 Saudi Arabia: ~Rs. 593,000–1,113,000/month 🔹 UK FY1: ~Rs. 1.21 million/month Beyond pay, doctors also face: ✔️ Long, exhausting duty hours ✔️ Limited training & residency slots ✔️ Workforce shortages with rising unemployment ✔️ Lack of career security and progression Migration is often not a choice of preference, but of necessity. A system cannot retain its doctors without fair compensation, adequate opportunities, and professional respect. Brain drain is not a doctor problem it is a system problem. Why #PayraiseForDoctors? Because in simple maths: 1 month salary of a doctor abroad 1 full year salary of a Pakistani house officer Pakistan house officer: PKR 69,600/month = PKR 835,200/year UK/Australia basic monthly doctor pay, when converted to PKR, can cross this yearly amount. This is not fair pay. Pakistan has only around 1.08 doctors per 1,000 people. That means 1 doctor for almost 923 people. Very bad ratio. Very dangerous for patients. And now trained doctors are being pushed out because of low pay, unsafe hospitals, burnout and no real future. Pakistan spends years training doctors. Then other countries take the benefit. This is not smart policy. A country with low doctor density cannot afford doctor migration. Pay them fairly. Retain them. Protect patients. @asfandyarbitani )³±

Pakistan is not a country that can afford to lose doctors. Population of Pakistan: 241.49 million World Bank/WHO physician density for Pakistan: about 1.08 doctors per 1,000 people That means roughly: 1 doctor for every 923 people And this is before we even adjust for: • doctors not actively practicing • doctors shifted to private sector only • doctors preparing foreign exams • doctors already migrated • rural areas with far fewer doctors So when even a few thousand doctors prepare to leave, it is not “personal choice” only.
