@lukha33 Two main considerations, ectopic ureter in duplex systems or ectopic ureterocele with no significant reflux in any other unit. Babies, less than several months I do open, but older kids we do lap, or robot assisted. The size of the upper pole ureter does not matter.

I worry more about lower pole injury during heminephrectomy than complications during UU. As long as one follows standard reconstruction principles, UU complications are uncommon. Plus it’s more enjoyable putting things together than taking them apart!

Steve Koff taught me that as long as distal ureter and bladder are normal, there should be few problems upstream.

Not a criticism but a True question because I know that’s how we think
