These issues are not isolated; they reflect systemic weaknesses that have persisted despite prior warnings. 1. Ineligible and Unsafe Providers Remaining in the System - Providers with suspended, restricted, or revoked licenses were not removed from Medicaid participation. - Some providers continued treating patients and receiving payments despite active disciplinary actions. - Cases included providers practicing outside permitted scope or under restrictions.   Impact: - Patient safety risks - Continued disbursement of taxpayer funds to unqualified providers 2. Failure to Verify Credentials and Certifications - Required credentials (e.g., DEA registrations) were not consistently verified. - The system relied heavily on provider self-attestation rather than independent validation.   Impact: - Risk of unauthorized prescribing - Weak control over provider eligibility 3. Weak Oversight of Ownership and Background Information - Ownership structures of provider entities were not properly verified. - Applications were approved without sufficient background checks or cross-referencing.   Impact: - Potential concealed ownership by disqualified individuals - Reduced transparency and accountability 4. Failure to Implement Prior Audit Recommendations - Similar issues were identified in earlier audits (including 2021 findings). - Corrective actions were incomplete or not sustained.   Impact: - Recurring deficiencies indicate systemic failure - Lack of accountability and follow-through 5. Continued Payments Despite Red Flags - Medicaid funds were paid to providers under disciplinary review or with license limitations.   Impact: - Improper use of public funds - Financial exposure for the state 6. Rapid Spending Growth Triggering Further Scrutiny - Significant increases in certain Medicaid service categories. - Oversight did not scale with spending growth.   Impact: - Increased risk of waste, fraud, or inefficiency 7. Policy Inconsistency and Unclear Enforcement Responsibility - Inconsistent interpretation and application of rules. - Reliance on external licensing boards instead of internal enforcement.   Impact: - Gaps in accountability - Inconsistent enforcement Key Systemic Themes Structural Weaknesses: - Inadequate verification systems - Poor data integration - Overreliance on self-reporting   Management Failures: - Ignoring prior audit findings - Inconsistent enforcement - Weak internal controls There's a pattern of systemic mismanagement within North Carolina’s Medicaid program.
