Thrilled to share our new paper led by Jeremy Chang @UIowa_Surgery and Dr. James Howe! @UIowaCancer @WinshipAtEmory Peritoneal metastases (PM) in small bowel neuroendocrine tumors (SBNETs) are uniquely challenging; patients face worse oncologic outcomes, and complications such as obstruction which further complicate care. Our approach: 🔹 Pre-op imaging with DOTA PET/CT + arterial/venous phase CT to map disease and guide surgery 🔹 Disease biology drives decisions — synchronous liver mets don’t exclude surgery 🔹 For G1–G2 well-diff SBNETs → aim for CC-0/1 and >70% cytoreduction 🔹 Even G3 well-diff tumors may benefit from surgical consideration 🔹 If cytoreduction isn’t feasible, palliative surgery or systemic therapy (SSAs, chemo, PRRT*) are options 🔹 Multidisciplinary collaboration is key PM in SBNETs demands nuanced, individualized, and team-based management — balancing oncologic control, symptom relief, and patient goals. 📖 tinyurl.com/mwhwf5e9 (PRRT may increase bowel obstruction risk)

The American College of Pediatricians just put out a 🔥🔥🔥 statement calling out all the major medical associations by name for pushing the gender transition craze on kids. They ask for these groups to "IMMEDIATELY stop the promotion of social affirmation, puberty blockers, cross-sex hormones and surgeries for children and adolescents who experience distress over their biological sex." The full @ACPeds statement ⬇️ "Therefore, given the recent research and the revelations of the harmful approach advocated by WPATH and its followers in the United States, we, the undersigned, call upon the medical professional organizations of the United States, including the American Academy of Pediatrics, the  Endocrine Society, the Pediatric Endocrine Society, American Medical Association, the American Psychological Association, and the American Academy of Child and Adolescent Psychiatry to follow the science and their European professional colleagues and immediately stop the promotion of social affirmation, puberty blockers, cross-sex hormones and surgeries for children and adolescents who experience distress over their biological sex.  Instead, these organizations should recommend comprehensive evaluations and therapies aimed at identifying and addressing underlying psychological co-morbidities and neurodiversity that often predispose to and accompany gender dysphoria. 🔥Hot off the press- Happy to share our real world post-marketing pharmacovigilance analysis on hematological toxicities (focus on MDS/AML) with Lutathera (Lu177-DOTA-TATE) in 🇺🇸 . 👉🏽Post-marketing surveillance data from the @US_FDA (2018-2023) 👉🏽 3443 adverse events, 243 (7.1%) were hematologic 👉🏽 majority thrombocytopenia, leukopenia, anemia and pancytopenia 👉🏽 MDS & AML comprised 2% of the reported events each. 👉🏽For context we compare the reporting of hematological toxicities (esp MDS and AML) with topoisomerase inhibitors 👉🏽This is a helpful reference that may potentially inform discussions with patients planning to receive PRRT (now referred to as radioligand therapy or RLT).
