PDPlan.org Key Points: - Nice comment in Lancet and series in Lancet Neurology. - ALS isn’t a single disease. - The most common genetic culprits are SOD1, FUS, C9orf72, and TARDBP. - Each genetic subtype paints us a different picture of how motor neurons may die. - Tofersen, targeting SOD1, may slightly slow progression in trials. - FUS-related ALS is showing response to jacifusen, an antisense therapy now in phase 3. - C9orf72 remains the biggest genetic piece of the puzzle and it accounts for ~40% of familial ALS. - TDP-43 pathology is present in nearly all ALS cases and tackling TDP-43 is a huge challenge. My take: Each subtype of ALS offers a lens into motor neuron vulnerability. Could therapies that fail in one potentially guide success in another? What do we need to do? Predict risk, slow onset and prevent disease. Should we consider universal genetic testing in ALS, regardless of age or family history? Every clue matters. Why should we care about the environment in ALS? Environmental exposures (e.g., pesticides, heavy metals, military related toxins, viral infections) may all possibly act as triggers or accelerators. Remember clues to cause, could also be clues to prevention. The environmental risk factors will likely help us to piece together why ALS develops and how we might prevent it, especially in the vast majority of people without known genetic mutations. Environmental exposures can affect how genes are turned on/off without changing the DNA sequence. We call this field epigenetics. Unlike genes, environmental exposures can be changed, regulated, or avoided.
