Diary of a Female Surgery Resident
This story stopped me in my tracks. Michael DeBakey invented the classification system for aortic dissection. He pioneered the surgery to repair it and trained hundreds of surgeons to perform it. At 97, when it happened to him, he refused the operation. He knew what the recovery would do to his body as he had seen it destroy patients. He signed a DNR and went home to die on his own terms, but the condition worsened. His wife overruled him. The ethics committee approved surgery after a long back and forth. The anaesthesia team refused to participate as he had been clear about his wishes, so another team was brought in. His own students operated on him using the grafts he invented, followed techniques he'd taught them, and they saved him anyway. He lived two more years. Went back to work, lectured, trained, delivered speeches, and received the Congressional Gold Medal. The man who taught the world to fight for every heartbeat had to be convinced at the end that his own was still worth fighting for. Sometimes, the hardest patient to save is yourself. A New Building for 76 year old Madras Institute of Neurology and now upgraded as Tamil Nadu Institute of Mental Health And Neuro Sciences or TNIMHANS All are welcome for the Opening Ceremony -oOo- 361 year old Rajiv Gandhi Government General Hospital, Chennai is a component institution of the 191 year old Madras Medical College started in 1835 Madras Institute of Neurology (MIN) was started in 1950 as a combined Institute of Neurosciences and modelled after Montreal Institute of Neurology (MIN). Dr. B. Ramamurthi, after his return from Newcastle, UK, in 1950, after undergoing training in neurosurgery under Professor G. F. Rowbotham, started the Department of Neurosurgery in the Government General Hospital on the auspicious Vijayadasami day (October 1950), with a few beds in the general surgical ward. The department was expanded later. Dr. S. T. Narasimhan, who had been trained in the United States, and had brought the first EEG machine to Chennai, joined Dr. Ramamurthi in the department. Cerebral angiography was introduced in 1952, with the joining of Dr. K. M. Pillai and Dr. M. G. Varadharajan, the neuroradiologists. Dr. V. Rajagopal, after undergoing training in neuroanesthesia in the United Kingdom, joined the department in 1953. Dr. S. Balaparameswara Rao joined the department as the first neurosurgery trainee and holds the unique distinction of starting neurosurgery departments at Vishakhapatnam and Hyderabad. Dr. V. Balasubramaniam, who was a brilliant surgeon, as well as Dr. M. Natarajan, subsequently became a part of the department. Dr. G. Arjundas joined the department as the first neurologist in 1957 after undergoing neurology training at Queen's Square, London. Dr. S. Kalyanaraman, after his neurosurgical training under Prof. Norman Dott and Prof. Gillingham at Edinburgh, also joined the department. The neurology division got strengthened further by Dr. K. Jagannathan, who joined in 1962 after neurology training at the Queen's Square, London. After that, the department came to be called as the Department of Neurology and Neurosurgery. Neurology as a super-speciality was the first neurology department in India, initiated by Prof. G. Arjundas and Prof. K. Jagannathan. The Institute was bifurcated into the Institute of Neurology and the Institute of Neurosurgery in 2015, with separate directors to enhance academic activities and patient care. Prof. K. Bhanu was the first Director of the Institute of Neurology, succeeded by Prof. S. Gobinathan and Prof. R. Lakshmi Narasimhan. The Institute is currently headed by Prof. Dr. S. Balasubramanian. The first DM Neurology candidate in India was trained at our Institute. Ours is the first institute in the country to offer the MCh Neurosurgery course. The new disease entity "Madras Motor Neuron Disease" was first described in our Institute by Meenakshisundaram E, Jagannathan K, and Ramamurthi B in 1970. In 1952, Neuroradiology was started. In 1957, Electroencephalography was introduced, and in 1962 a full Neurology unit was inaugurated. In 1966, the PhD Programme and DM Neurology course were initiated. In 1967, the Neuro Ophthalmology department was started. In 1968, the foundation stone for a separate building of the Institute of Neurology was laid. In 1970, the Neuropathology department was established. In 1972, the Institute of Neurology was officially declared open. In 1975, the Silver Jubilee celebration was conducted. In 1977, DM Postgraduate seats were increased to 5. In 1980, the CT Scan was commissioned. In 1983, Speech Therapy was started. In 1984, Evoked Potential study was introduced. In 1990, the Headache Clinic was started. In 1991, the Epilepsy Clinic was initiated. In 1993, the NSI Annual Conference was organized. In 1997, the 5th Annual Conference of the National IAN was organized. In 1998, the Movement Disorder Clinic was started. In 2000, the Golden Jubilee Celebration of the Institute was conducted. In 2008, the Golden Jubilee Celebration of the Neurology Department was celebrated. In 2010, DM postgraduate seats were increased to 14. In 2012, the Stroke Ward was inaugurated. In 2012, the first IV Thrombolysis for Acute Ischemic Stroke in a government institution in our state was started in our institute. In 2020, BSc Electrophysiology courses with 10 students were started. In 2021, the Gait Lab was started. In 2022, Video Nystagmography was introduced. In 2022, the Stroke ICU was started. In 2023, the Autonomic Function Lab was instituted for the first time in a government institute in our state.

Myocardial Performance Index (Tei Index)= Interesting one 😍 The Tei Index, named after Dr. Chuwa Tei is a Doppler-based, noninvasive measure of global ventricular function, combining both systolic & diastolic performance. How it’s measured: ⬜ Obtained using pulsed and continuous wave Doppler of mitral inflow (A4C view) and LV outflow (A5C view). ⬜ Key time intervals are measured: a. IVCT ➡️ Isovolumetric Contraction Time b. IVRT ➡️Isovolumetric Relaxation Time c. ET ➡️ Ejection Time Formula: Tei Index = (IVCT + IVRT) / ET or Tei Index = (a - b) / b a = time from mitral valve closure to opening b = aortic ejection time Normal Values: LV Tei Index: ≤ 0.40 RV Tei Index: ≤ 0.43 Interpretation: 🔵 Higher Tei Index = Worsening ventricular function 🔵 Sensitive for detecting early dysfunction, even when LVEF is preserved. Clinical Significance: - Helpful in evaluating cardiomyopathies, heart failure, congenital heart disease, and transplant rejection. - Valuable in both pediatric and adult cardiology. From being batchmates at Bangalore Medical College to soulmates celebrating our 22nd Valentine’s together ❤️ ( my first valentine day gift in 2004 final MBBS was ECG pocket Manual 🫢) Thank you, @AparnaSreevatsa, for tolerating all the tantrums and mood swings over the years. One thing I’ve learned , two people can have completely different opinions and still build a beautiful life together. I’m the fiction person; she’s pure non-fiction. I’m talkative; she’s calm and reserved. I’m a set dosa person; she’s a crispy masala dosa loyalist. I love cars; she believes money has better destinations. Despite these minor trifles (and the occasional car debate 😬), we’ve always chosen to stick together for better or worse. 22 years later, still learning, still arguing, still fighting uppinakaayi Vs Paapad as side dish 🤣. Happy Valentine’s Day to all you lovely people here. Don’t let ego come in the way of love. It’s rarely worth it. ❤️
