@generalorthomd Tough case, I would revise the fixation with a fibula allograft, or i would nail it and open on the nonunion site, burr it and pack it up with iliac crest autograft maybe even tricortical.
I would shy away from arthroplasty, due to increased postop complications risk in his case

Not sure I can call this “Hypertrophic”…it tried, and failed to heal. Oligotrophic…kindatrophic, notquiteotrophic.

60M, recently dx Lewy Body Dementia, Parkinson’s sx currently. No cognitive decline.
Came to me 3 months after initial injury that had partially healed then new injury refractured. Now 6 months post op ORIF with hypertrophic nonunion.
Infection workup negative.
