Can you spot spinal segmental myoclonus? This great video case by Ruiz-Ortiz and Benito-León in @tremorjournal shows it manifesting in the setting of multiple sclerosis and focal inflammation of the spinal cord. The video reveals 'involuntary, arrhythmic, jerky movements localized to upper trunk + proximal left arm. Possible mechanisms: 'axonal hyperexcitability, spontaneous neuronal discharge, disinhibition of alpha-motor neurons and disruption of spinal interneuronal circuitry.' It's tough to treat and folks have tried clonazepam, levetiracetam, depakote, and tizanidine. tremorjournal.org/articles/10.53… #myoclonus #movementdisorder #parkinsons
