“But bread causes my symptoms…” 🍞🤔 How should clinicians approach this common consultation? In @FrontGastro_BMJ, this practical piece explores possible clinical scenarios behind bread-related symptoms — from coeliac disease to wheat allergy, IBS, FODMAP sensitivity and beyond. 🔎🩺 A useful read for turning a familiar patient concern into a structured, evidence-based conversation. 🥨 🥐 ‘But bread causes my symptoms’: possible clinical scenarios 🍞 🫚 Read this fantastic review paper by Francesca Manza and co-authors! 🍞 Wheat components can induce reactions in a number of people. ⚛️ Gluten is the known trigger in coeliac disease, mediated by human leucocyte antigen-DQ2/DQ8 and adaptive and innate immune responses. 🫚 Wheat allergy involves immunoglobulin E-mediated reactions to specific wheat proteins such as gliadins, lipid transfer proteins and albumins. WHAT THIS STUDY ADDS Non-coeliac gluten/wheat sensitivity is often overlapping with irritable bowel syndrome as it presents with shared intestinal and extraintestinal symptoms but lacks clear biomarkers, making diagnosis reliant on exclusion and food challenges. HOW THIS STUDY MIGHT AFFECT RESEARCH, PRACTICE OR POLICY Diagnosis of wheat-related disorders can be challenging in a clinical setting and requires a stepwise approach. fg.bmj.com/content/early/… @PhilSmithIsBack @OTavabie @dr_aditi_kumar @TrevorTabone @eathar_s @IrenePerezMD @KGananandan @zare_benjamin @medicalreg @dtleiberman #wheat #gluten #coeliacdisease #celiacdisease #glutenfree #foodallergy #foodintolerance #autoimmunedisease #gastroenterology #digestivehealth #guthealth #ibd #gihealth #nutrition #malabsorption

🌾 Recent findings indicate that 10% of people worldwide self-report non-celiac gluten or wheat sensitivity. But gluten may not always be to blame for symptoms of this poorly understood condition. #GlutenFree ja.ma/4nZPCXd

🥗 Mediterranean diet beats standard advice for IBS In a UK RCT, 62% of IBS patients improved on the Mediterranean diet vs 42% with traditional dietary advice — showing superiority and noninferiority. 👉 1st-line option beyond low-FODMAP. pubmed.ncbi.nlm.nih.gov/41144975/ #GITwitter
