Few weeks ago, I had the honour of meeting Her Majesty The Queen at @ucl in London. Thank you @JDRFUK for the kind invitation. It was an honour to participate and speak to Her Majesty.

It takes an average of 17 years for evidence to change practice. Shocking stat from #ATTD2026. To put it simply, some are implementing advice from #ATTD2009 in 2026. Huge disconnect which needs addressing. Thanks for raising @parthaskar

A snap from recently concluded @ATTDconf One of my favourite and oft used slides I never get tired of saying it. Access of science in #Diabetes should be for all- and we should ALL work to do that The science is worth nothing if the person its intended for? Doesn't get it. @BT1DUK @DiabetesUK @BreakthroughT1D @lifeforachild @TBattelino @kamleshkhunti

The Core Problem: LLMs generate confident-sounding dosing advice rather than admitting they don't understand the data. My take: We need to control AI outputs by building custom models built exclusively on private, high quality data not the internet! Expert agents.

Phenomenal session by @Tims_Pants on AI in diabetes care at #ATTD2026. Some of my key takeaways: • Generic LLMs (ChatGPT, Grok, etc.) cannot safely provide insulin dosing recommendations. • When given weeks of CGM data, Al skips over large chunks and fabricates summaries.

Did you know? As per data Diabetic Keto Acidosis rates in those previously diagnosed with #T1Diabetes is now the LOWEST since records began @NHSEngland Across all ages A plateau with a change in pattern since 2018 Across a country & health system How. About. That. #gbdoc

You’re allowed to not run diabetes tech at maximum capacity. Choose sustainability over perfection. This is what I’m exploring in my new YouTube video. This video is about choosing what earns your attention. 👇 Watch the full video and learn more here: youtu.be/1XaH5u6Gx_E

@Kerr21Kerr So the Simplera app works in the background but just doesn’t show the glucose when you open it?

@nerdabetic The InPen App is connected to the Simplera App via the cloud (so is my watch repeat) so the App must be working just won’t let me get to it - when I need to change the sensor tomorrow it might be a call to the helpline if it doesn’t work then.

@Kerr21Kerr Ohhh I see. Is the actual inPen app connected to the CGM then not the Simplera app?

@nerdabetic It always has before (logs you out at least once a month) but not today but the CGM feed is still going to the InPen App and my watch so I can see values just not able to use the main Simplera App (and no alarms) - very weird

@Kerr21Kerr Oh no way? So it doesn’t pick up the existing sensor session???? Even when you log back on?

@nerdabetic So today’s tech issue is the Simplera App has logged me out and when I log back in again the only option it offers is to insert a new sensor but the system is still sending the current data to my InPen App - sensor change should be tomorrow….

@Kerr21Kerr “New” texh category is so tricky to distribute right in the UK. I think GP via FP10 is the right approach, especially with most of them being very familiar with the Libre sensor and how it works. It’s just an extension of that with a smart pen.

@nerdabetic Seems Medtronic are hoping to get InPen and instinct sensors onto FP10 at some point so it can be prescribed by GPs although I think for most some training would be useful to take full advantage (and it needs inputs similar to a pump for the calculator to work correctly).

My diabetes tech “budget” mostly goes to: • bolusing for big meals • training at the gym • checking trends when I need them Alarms didn’t make the cut — and that’s intentional. Watch my new video: The Hidden Cost of Diabetes Tech: Your Attention youtu.be/1XaH5u6Gx_E

@Kerr21Kerr Thank you! Yes — unreliable sensors are one of the biggest hidden drains. InPen solution sounds great on this front for you! Finding that balance is tough and it changes with time.

@nerdabetic I like the tech budget concept - sadly unreliable sensors take a big bite of mine! MDI with regular exercise means fall alert is helpful plus missed bolus alert - InPen system with IOB is an improvement on Libre basic alarms and I don’t feel the need to check numbers as often.
