The MCCD should only include relevant contributing factors. It is not a discharge summary and shouldn't include their entire medical history.

If you want to propose a cause of death but are unsure don't tick the cause unknown box. If it's wrong the medical examiner or the coroner will correct it.

Our next session is from Dr Rachel Craig who is a lead medical examiner for Barnet. She is also a palliative care consultant. She will be talking us through how to fill in a medical certificate of cause of death and some of the changes that have been introduced due to Covid19

In conclusion it is important to remember that inquiries are fact-finding not about apportioning blame. If you're concerned about an inquiry speak to your supervisor, your trust medical examiner or legal advisor or your defence union.

The press may attend inquiries. It is important to remember that the duty of confidentiality applies once you have finished giving evidence. Questions from the press should be referred to either the trust or your defence union press office.
