A culture of learning and candour has transformed safety in flying (not a single commercial flight death last year) and is a priority for all healthcare systems.

No one is above the law. But nor is the law above reproach - which is why we have law reform. The fad for regarding a mistake as a crime is actually new. Criminality always required intent. It is iniquitous as well as dangerous to criminalise trying to save your life.

Blame is inimical to patient safety just as it is to airline safety. We need candour not defensiveness, learning not criminalisation of mistakes.
