Just finished an online consultation with the mother of a 3 year old boy (with no disease) from Gujarat (for height & weight issues).
The mother told me she hasn’t given her child sugar till now.

I was shocked and asked, “Why??”
She said, “The paediatrician told me not to give sugar.”

I was stunned and told her to start giving sugar immediately.
Doctors nowadays seem to have everything except common sense, and parents seem to have lost it too.

Brainstem reflexes are crucial in neurological exams and coma assessment.
Each reflex tests a specific cranial nerve pathway and brainstem level.
Here’s a simple breakdown every clinician should know 👇
1️⃣ Pupillary Light Reflex
Afferent → CN II (Optic)
Efferent → CN III (Oculomotor)
Level → Midbrain
Test: Shine light into one eye
Normal → Both pupils constrict (direct + consensual)
❌ Absent in:
• Optic nerve damage
• Oculomotor palsy
• Midbrain lesion
• Severe hypoxia / brain death
2️⃣ Corneal Reflex
Afferent → CN V1 (Trigeminal)
Efferent → CN VII (Facial)
Level → Pons
🩺 Test: Lightly touch cornea with cotton wisp
Normal → Bilateral blinking
❌ Absent in:
• Trigeminal lesion
• Facial nerve palsy
• Pontine lesion
• Deep coma
3️⃣ Vestibulo-Ocular Reflex (Doll’s Eye)
Afferent → CN VIII (Vestibular)
Efferent → CN III, IV, VI
Level → Pons + Midbrain
🩺 Test: Turn head quickly (only if C-spine safe)
Normal → Eyes move opposite to head
❌ Absent in:
• Brainstem injury
• Deep coma
• Brain death
4️⃣ Oculovestibular Reflex (Cold Caloric Test)
Afferent → CN VIII
Efferent → CN III, IV, VI
Level → Pons + Midbrain
🩺 Test: Cold water into ear canal
Normal → Eyes deviate toward irrigated ear
Mnemonic → COWS
Cold → Opposite (fast phase)
❌ Absent in severe brainstem dysfunction
5️⃣ Gag Reflex
Afferent → CN IX (Glossopharyngeal)
Efferent → CN X (Vagus)
Level → Medulla
🩺 Test: Touch posterior pharynx
Normal → Gagging / pharyngeal contraction
❌ Absent in:
• CN IX or X lesion
• Medullary damage
• Deep coma
6️⃣ Cough Reflex
Afferent → CN X
Efferent → CN X
Level → Medulla
🩺 Test: Tracheal suction
Normal → Cough

Brainstem reflexes help localize lesions:
• Midbrain → Pupillary reflex
• Pons → Corneal + VOR
• Medulla → Gag + Cough
Loss of all brainstem reflexes = Brainstem failure.
