They seem to forget that the most specialised medical treatments in South Africa are often carried out in public hospitals. They also forget that even with medical aid, patients are sometimes transferred from private hospitals to public ones for specialised treatment, as public hospitals have the most experienced doctors and the most advanced facilities for complex cases.
