If you’re able to match into ortho in the US it means you are extremely intelligent. So: trust your own ability to reason and logic MORE than the guidelines you’re forced to memorize. When something doesn’t seem right, no matter what the source is (attending, mentor, textbook, uptodate, boards, AI, NEJM, whatever) keep digging until you make it make sense for YOU, even if you end up concluding that everything you were taught was wrong.
Do it in a state that honors medical autonomy, where patients can refuse any medical intervention they want without risk of facing mandates and doctors can go off guideline with minimal regulatory risk.
Stay true to your oath, the original reason you chose medicine: to leverage all the available medical science for the ultimate benefit of the patient, every time. Keeping your integrity with your patients is worth more than any paycheck or any title.
Always be skeptical, always look for sources of bias and manipulation in the information that is being presented to you. There are many forces that want to profit off of manipulating the way you make decisions.
Be open to working collaboratively with patients and colleagues (including non-allopathic health practitioners) that have a more integrative perspective than you do. They can teach you a lot if you’re open to it.
Remember that the US institutions have the most prestige but the US is not the end all and be all of medical treatment. There are many niches (example in your field: articular stem cell therapies) where international facilities are way ahead because of more open regulation and more appreciation for integrative approaches.
Read. A lot. Much of what you have been taught and will be taught is just plain wrong.
The biochemistry, molecular biology, pathology, etc, basic science of the first two years of med school, even though your attending a basically ignore most of that, if you can learn how to deepen and leverage that basic medical science for the benefit of your patient, you will serve them in a much more long term beneficial and truly scientific manner than your colleagues who just follow the guidelines without thinking.

It’s important to understand that most people who debate on this platform do not do so with genuine intent to understand reality and learn from the exchange.
They simply are trying to farm engagement and use any disingenuous rhetorical trick they can conjure, including stories, tales, lies, exaggerations, mischaracterizations, playing dumb (which is the one I hate the most), deflecting, distraction, strawman, ad hominem, inventing restrictive rules of discourse on the spot, and on and on.
When faced with facts they cannot refute, they try a few of their tricks. A favorite trick of these types lately is to just regurgitate some slop they got from AI. Due to their dim IQ, they themselves are so awestruck by AI that they think that no one could possibly effectively refute the garbage that AI pumps out. They are left flabbergasted because:
1. Overuse of AI has worsened their cognitive deficit
2. They don’t understand how someone can be more intelligent or more forthright than AI
3. They find themselves exposed for pretending to be an expert with the ventriloquist AI and not knowing how to respond
They inevitably block because they were never interested in genuine debate and learning and knowledge sharing for their audience, and they want to insulate their audience from the criticism and logic that is being presented.
These people have PhDs, MDs, JDs, executives at major institutions, and they are found to be completely lacking mentally. They are just political propaganda tools with bifocals.
Beware who you get your information from. The tools for propaganda and narrative control are pervasive, and in this short form video low attention span low literacy world we are in now, people don’t have the patience nor the capacity to process and suss out truth from half-truth.
Don’t be like Sanjeev. Stay open to new information, even if that means realizing you’ve been wrong and that you’re out of your element and that there are experts that are smarter and more informed and less politically handicapped than AI. Don’t block people you disagree with if it is simply a difference of perspective or opinion or interpretation. When you’re wrong, admit it. Keep an open mind. If we’ve learned anything from the last 10 years, let it be that the “truth” is rarely settled and that the establishment and “expert” narrative is riddled with bias and propaganda and psychological operations that can take decades to centuries to sort out by bold intelligent individuals who are willing to stand against the whole damn world for the defense of the truth.
