my quick thoughts on the walmart primary care stuff 1) it's virtually impossible to make primary care economics work unless it's a loss-leader to other services or takes risk. Maybe one of the reasons CVS pushes forward while all the others push back is because of the attached payer (Aetna) 2) It's doubly hard when your company is evaluated by retail metrics instead of healthcare (e.g. sales per square foot) 3) honestly I think one of the only ways we're going to change rural healthcare is by totally expanding scope of practice for pharmacists/caregivers to do way more clinical things (maybe with AI-assisted tools) because part of what makes these models so untenable is staffing costs for clinicians (see @dp_oneill tweet x.com/dp_oneill/stat…) 4) I think it's easy to look at the telemedicine companies shutting down like Optum, Walmart but almost all of them have implemented some of the worst version of telemedicine - taking the same face-to-face modality and putting it online. I think maybe this is the end for urgent care telemedicine as a business, but I think a risk-based chronic care management (telemedicine + in-person) is still totally untapped 5) It's ...sad? I think of all companies Walmart had the best chance of delivering differentiated primary care in rural areas. I hope another company with a strong rural presence tries again
