@ameerfarooq @rbarbosa91 We also do IOCs in just over 50% as a group - selective IOCs. Still not sure how routine helps. I think the never IOCers are missing out though for when they need one

Everyone's got their own bias. However routine IOCs already had their era - we have pretty good evidence they dont help the average surgeon delineate anatomy and the data arpund IOC is about 20 years old.... Im not opposed to saving patients from unnecessary ERCPs but routine IOCs dont make sense. One of the few times I disagree with the great Ron Barbosa!

“I’m not doing more IOC’s. They cost more, take more time, and there’s no proven benefit“, says the surgeon. “They’re not going to tell me how to practice surgery.“ <turns to rep> “OK, I’m ready to dock the robot.“

Am I the only one who thinks that this is an absurd recommendation? Routine Intraoperative Cholangiogram for Elective Laparoscopic Cholecystectomy Is Now Recommended facs.org/for-medical-pr…

@docmartin22 No clinically relevant difference in recurrence should have been the take away.

Alternative headline: analysis that captures none of the clinically relevant factors for hernia recurrence shows no difference in recurrence rates. 🤷‍♂️
JAMA Surgery@JAMASurgery

Robotic-assisted groin hernia repair was associated with a higher long-term operative recurrence rate than laparoscopic or open repair among #Medicare beneficiaries, with overall recurrence rates remaining low across all methods. ja.ma/3QEoF05

When somebody finally publishes a study validating that thing you’ve been saying on rounds for the last 15 years, and you don’t know how to feel about that.

Devils in the details: strict selection criteria ("to enable the selection of patients most likely having a single adhesive band"), study population average BMI <25, <10% had DM, no mention of Tob. Cool study, but this doesn't reflect my average patient...give me laparoscopy!
JAMA Surgery@JAMASurgery

Laparoscopic adhesiolysis for #SmallBowelObstruction was not superior to open surgery for long-term recurrence, incisional hernia incidence, or quality of life at 5-year follow-up, confirming both as viable options. ja.ma/4c6H4eO

More in-line with surgeon work related issues are talks at @PCSAsurg : A large @AmCollSurgeons survey of surgeons reported by @UCSFSurgery team and medical student Saffanat Sumra addressed surgeon workload. The greatest predictor of feeling overworked or burnt out is based on years in practice (pivots at 30yrs), and then #hours worked (pivots at 66hours 😮 in under 30yrs and 45 hours in over 30yrs in practice). Anyone else agree? And why are we abusing our surgeons so much? #PCSA2026

Surgeons: Noticed you hurt after a day in the OR? @pihaigh and his team evaluated this nicely on @PCSAsurg . Results? - every part of the body hurts more after an OR day than non-OR day - robotics has more wrist pain - laparoscopy has more upper back pain - open surgery has more lower back and neck pain How can we right this wrong? #PCSA2026
