Dr. Amy Proal in collaboration with the CoRE Clinic this week: “A big goal of ours is that [Long COVID] diagnostics are available to be ordered in insurance care for the most number of patients. You have to choose a different regulatory route and that requires a lot of interactions with the FDA, a lot of validation experiments… This incubation process of getting the diagnostic out of academia and into the real world, that’s something we’ve been working on a lot.”

The major focus of this trial is to determine which LC patient subtypes benefit most from the drug. 2/ PolyBio is supporting this project investigating the oral cavity as a window into the identification of Long COVID immune, clotting, microbiome, and viral protein biomarkers. The team is using shotgun proteomics to identify persistent innate immune and clotting dysfunction in saliva collected from Long COVID patients and matched controls. Similar analyses are also being performed on patient blood to determine the degree to which Long COVID saliva biomarkers mirror those in the systemic circulation. polybio.org/projects/long-…

Dr. Marcelo Freire and his team are leading efforts to implement Salivary Proteomics into Long COVID clinical trials as primary endpoints. In a recent Mount Sinai-based trial of Vagus Nerve Stimulation for LC, this noninvasive method suggested a significant difference in inflammatory responses between treatment and sham groups over 12 weeks. Persistently upregulated salivary proteins in the sham group included serum amyloid A4 protein. “The clinical significance of proteins like these present in saliva is… it corresponds to an increased relationship in rheumatoid arthritis, oncology & atherosclerosis,” said Freire. Whereas the active VNS treatment group demonstrated consistent downregulation of several inflammatory markers, including gamma-synuclein, sometimes connected to neurodegenerative disease, as well as breast & colorectal cancer.

The team is continuing to validate T cell activation via [18F]F-AraG as a potential biomarker of inflammation in clinical trials within PolyBio’s network, including the current UCSF-based trial of Ensitrelvir for Long COVID. This research will help determine the degree to which the therapeutics are successfully lowering Long COVID-associated inflammation. 3/

Dr. Henry Van Brocklin and his UCSF team are investigating the novel radiotracer [18F]F-AraG as a biomarker of long-term immune dysregulation in Long COVID. When paired with PET imaging, the tracer has shown persistent T cell activation in tissues such as the brain, lungs, gut, spinal cord & bone marrow. “Even in cases where the Long COVID symptoms were not associated with the [organ] phenotype, we actually saw elevation there,” said Van Brocklin. The method is actively being tested in a clinical trial of Ensitrelvir for Long COVID to determine measurable changes in immune activation following treatment versus placebo.

The PolyBio-supported research project discussed by Dr. Pretorius is investigating the degree to which viral proteins or bacterial products contribute to the composition of Long COVID fibrinaloid microclots. Fibrinaloid microclots are being isolated from the blood of Long COVID study participants and assessed via metatranscriptomic sequencing and advanced proteomics. 2/ polybio.org/projects/analy…

Said Dr. Resia Pretorius on differentiating microclot components between Long COVID, ME/CFS and POTS: “If we can understand the protein content associated with these amyloidygenic molecules, we think we might be able to identify, in that [insoluble plasma] fraction, proteins that we can target. “In this proteomics analysis specifically, we are finding a lot of post-translational modifications.
