46 yo M w/ HTN, dyslipidemia, active smoking w/ new onset class III angina despite 2 anti-anginals. CTA w/ significant proximal LAD disease. Cath (films below) shows ostial LAD CTO with large epicardial collateral from RCA. No other sig. lesion. Nl. EF. See poll. @SachinGoelMD
