@ChampionsOTIce Stalker did it nicely by reducing everyones health and bumping up gun damage It reduces how many "mistakes" any given actor can make

Hard modes should, ideally, add more complicated features. Enemies that normally use pistols and SMGs suddenly start using grenades or launchers. Enemy heavies start spawning accompanied by support units that can block incoming damage unless you take them out first. And of course there's always old reliable: increasing the number of enemies while decreasing available ammo. Yes, the enemy still dies easily if you manage to get a shot on target, but now there are twice as many snipers as usual and they're covering each other.

A good way to avoid this is to have the bad guys be inhuman. Case in point: A Wendigo Cult Thrall vs a Confederate Militiaman Sure, the Thrall has much cooler gear. But you're going to cheer when the grubby old redneck shoots it in the face with a wasteland musket and hangs it from a lamp post.
