Obstetric cardiac arrest Let’s look at the big picture…. So among 355,000 obstetric anaesthetic interventions in a year in UK (delivered to approximately 50% of all births & at all times of day and night almost exclusively in the NHS) Incidence of obstetric cardiac arrest -Epidural for labour 1 in 56,500 - any anaesthesia for LSCS 1 in 8,500 -All epidurals & spinals 1 in 17,000 -all GAs 1 in 1,200 These fall into ‘big data’ They are no substitute for an RCT in assessing safety (except no such RCT exists or is likely to) There will clearly be confounding by intention (the very highest risk cases more likely to have GA) …..but they do provide welcome evidence of the absolute safety of obstetric epidurals& spinals (& possibly relative safety compared to GA…) I think a credit to UK obstetric anaesthesia & wider surgical & midwifery teams. …-publications.onlinelibrary.wiley.com/doi/10.1111/an…

Sincere words of affirmation don't take much effort & have a disproportionate impact on people’s self-esteem. As leaders, we should never underestimate the power of our own influence. A literature review commissioned by the @CareQualityComm on improvement culture in health and care settings. Overall, the review identified multiple positive outcomes associated with a "good" improvement culture including: 1) Increased responsiveness of services 2) Shorter waiting times 3) Better staff retention & lower use of agency staff 4) Better quality of care 5) Lower mortality rates 6) Treating people who use services with more dignity and respect 7) Improved staff morale and motivation 8) Increased collaboration between services 9) Better ratings from regulators 10) Improved financial outcomes ie, many reasons why we should prioritise the so-called "soft stuff", combined with an improvement methodology: cqc.org.uk/sites/default/….
