@Halalmin Guess he is never planning to be a consultant himself. Otherwise it is a bit of an own goal to demand this of consultants whilst training to be one.

Fails to realise that all consultants have done at least a decade of night shifts and also fails to understand the training hospital model. If you want consultants around the clock on the shop floor it's expensive, doesn't exist in a publicly funded model.

Most universal healthcare systems train competent doctors in 5–6 years. Only the NHS stretches “training” to 8–11 years by using juniors to plug rota gaps and cover service work that has nothing to do with learning. That’s not higher standards — it’s a structural choice that’s now breaking the pipeline.

@AriReddy @drnikkistamp Australia is moving to a PGY1+2 prevocational training pathway from 2026. No one enters specialist training in PGY 3 with the exception of general practice & psychiatry. Most enter in PGY3-4. Then add 5-7 years specialist training. Most become specialist around PGY10.

As I said earlier, the Australia figures I used came from official written sources. Hearing from you and others has made it clear that the real world pathway is longer than the published one, and I have already acknowledged that. I am not sure what more you expect from me on that point. It is also a tangent that does not materially change the underlying argument. Whether Australia has its own timeline inflation only reinforces the broader point, which is about the UK’s structural use of prolonged junior roles and post-completion limbo. That remains the central issue.

1️⃣ Most people don’t realize how unusual the UK’s doctor-training model is until they compare it internationally. France, Germany, Australia, the Netherlands — all move doctors to independent practice years faster. The UK isn’t the norm. It’s the outlier.

Building bigger hospitals without addressing the inefficiencies of an inpatient stay is an expensive way to just perpetuate the same problem on a larger scale. Early specialist review on Day 1 of hospital admission would reduce LOS, improve care & reduce low value tests

@RCEMpresident @RCollEM We need to broaden our view to the entire hospital as opposed to focusing on ED. When beds are finite we need to focus on efficient IP stays under the correct teams as opposed to anywhere from ED will do. We need more direct admission pathways and early specialist input

If insanity is repeatedly doing the same thing and expecting different results … Overcrowding in EDs kills patients and harms staff. The core problem is the “back door” of our departments. Continued focus on the “front door” through redirection strategies won’t work. @RCollEM

Currently dealing with a medical issue where I need to see a doctor. My local medical centre is closed today being Sunday, so gave one of Albo's Urgent Care Clinics a call. Asked what the wait time was. 3 hours. Australia is declining into third world standards.

Still thinking about the pace the Fever played last night. It was chaotic & frantic, yet somehow controlled. If they can replicate that tomorrow, they win. This hinges on rebounding. Tire out their bigs. I’m excited.
