@mmamas1973 How do you know the black person is underprivileged? Also how can you assume that there are no Asians who aren’t underprivileged? They also can come from poor disadvantaged backgrounds. Race shouldn’t be a factor in who is rich and who is poor.

@s_brownie Are you talking fellowship or attending jobs? H1B is never a fellowship friendly visa. Most fellowships are on J1. After that you cant get hired to any job, you have to commit to a 3 years federal waiver job in rural area on H1B before you are allowed to adjust status.

This is pure rage bait. The people who get frenzied about H-1B assume that tech-sector logic applies to medicine. It does not. For the sake of outrage and engagement, they frame it as though a hospital is bringing in someone from outside the country to take a job from an American, and that is simply not how medicine works. The best gastroenterologist in Canada or India cannot simply show up in the United States, get an H-1B, and take this job. They would still need to complete about 7 years of U.S. residency and fellowship training before they could even be eligible for a position like this. This is an April posting. They likely need to keep the H-1B option open so they do not end up empty-handed by July. Clearly, they are struggling to fill this position. The pool of candidates for a role like this is extremely limited. Lets get to the basics first: 🔴To become a licensed pediatric gastroenterologist able to do advanced endoscopy in the United States, you are looking at about 15 years after high school: 4 years of college, 4 years of medical school, 3 years of residency, 3 years of pediatric GI fellowship, and often 1 year of advanced endoscopy fellowship. 🔴There are only about 60 pediatric GI fellowship programs in the United States, graduating roughly 200 fellows each year. After that, advanced endoscopy is even narrower, with only about 15 programs nationwide graduating roughly 40 to 50 candidates proficient in advanced procedures. 🔴Then add another reality. Because compensation in general pediatrics is relatively low, pediatric residency relies heavily on international graduates. Roughly 30 to 40 percent of pediatric residency positions are filled by international graduates, while many U.S. graduates choose orthopedics, surgery, dermatology, and other higher-paying specialties, especially given student debt and the length of training. 🔴Even among U.S. graduates who do choose pediatrics, many stop after the 3 years of residency. The additional 3 to 4 years of fellowship training often do not offer a proportionate financial return. A pediatric gastroenterologist who spends 7 years in pediatric training after medical school, and a general pediatrician who trains for 3 years, can still end up in a similar salary range, often around $180,000 to $300,000 a year, despite the additional years of training and lost income. 🔴On top of that, about 30 to 40 percent of pediatric GI graduates are international graduates who require visas. And not all visa holders are equally available. Some require J-1 waivers, which may limit where they can work. So when you put it all together, if UPMC wants to fill this position, they are effectively competing for a pool of about 35 to 40 new graduates, nearly half of whom are visa-requiring. Narrow that further, and they may realistically be competing for something like 20 U.S. MD pediatric GI graduates plus about 10 H-1B-capable candidates. 🤷🏻 Now ask yourself: do you really think those 20 U.S. MDs are going to take a job that requires driving hours into rural Pittsburgh for lower academic pay? Of course not. Many will choose private practice or remain in major cities like Los Angeles, New York, San Francisco, or Houston close to their family members. And for academic careers, they have options at places like Harvard, CHOP, NYU, Hopkins, Nationwide, Rainbow Babies, and Mayo Clinic. UPMC is a mid tier academic place. They will also choose urban positions with leadership or research opportunities, not outreach-heavy roles. That is the point. If UPMC does not keep H-1B as a backup, with a pool of perhaps 25 viable candidates, they risk having no one in place by July. And the fact that this job is being posted this late in the cycle tells you exactly what is happening: they are not being selective. They are trying to avoid ending up with nobody.

Yeah this always happens. Celebrities cutting lines everywhere! Few years back, same thing happened to me. I’m standing at the gate, minding my own business, and the staff keeps telling me “sir please move aside quickly, very important person coming.” I was NOT going to put up with this bullshit. I decided that whoever this VIP is, I’m confronting this nonsense and got ready with my camera. Someone whispered: it’s a very famous cricketer. My brain immediately went: Sachin Tendulkar. And I thought… okay maybe I’ll let this one slide. But then the entourage arrived. Police everywhere. And out walks… Shikhar Dhawan. Bhai, all that security for SHIKHAR DHAWAN?! The fight mode came RIGHT back. So I marched straight up to him at the check-in counter. Looked him dead in the eyes. And said: “Sir, ek selfie milega?”

A 249–250 on Step 2 CK is considered average in today’s landscape, but for non-US IMGs, it often isn’t enough. Even scores in the 260s are no longer a guarantee. Without strong US clinical experience, meaningful research, and the right mentorship or connections, matching remains an uphill battle. In contrast, for US-IMGs, AMGs, and DOs, a 250 opens doors across many specialties. Same score, completely different reality.

I was in this boat before but I'm looking to it by a different way. First, you are getting trained on how to do research, so you should appreciate the opportunity you have. Second, if you put yourself in comparison with the AMGs, they pay around $300K to get into the residency training while you are paying nothing to be there. As an IMG, I feel like we are in far more better situation from financial point of view if I compare myself with an AMG! Just be grateful for the opportunity you have and try to show your good work to your professor so they can pay you faster!!
