This is my 3rd #tavr in TAV with this phenomenon of the Sapien jump. not a PVC, it is a frame-frame interaction even at 180 bpm : 26s3 in a 29 Evolut . In this case plenty of STJ space with higher implant . But have sequestered 1 left sinus due to the jump.

Obvious bias because Impella not available for me, but rota mandating Impella is craziness. Data so far for me equivocal at best, possible harm. Times I’ve wished had Impella were acute ischaemic severe systolic LVF.

@shahrukh_bakar Distinguishing features are inappropriately low troponin for degree of LV dysfunction, prolonged QTc and widespread TWI rather than what would be expected ACS and history often makes TTS almost certain.

@shahrukh_bakar Have a few times now seen ACS causing takotsubo. This clear TTS ventricle. Of course you can have TTS with severe CAD, not like CaD prevents TTS

Chest pain with hsTnT 1000. Precordial TWI. Extreme emotional stress within last 24 hours. Stress-induced cardiomyopathy?

@shahrukh_bakar Yeah exactly. The best example I can remember is a lateral STEMI with occluded OM and clear TTS ventricle. All the TTS recovered within days leaving inferolateral hypo.
