I’m of the opinion that almost every physician with federal student loans should file a tax return extension.
Every year.
Unless you’re an attending who’s not going for PSLF. And there’s one other exception I’ll expound on further down this post.
But if you’re in training (PSLF or not), doing so can help keep your payment as low as possible, helping you manage your budget.
If you’re out of training and going for PSLF, it can help maximize loan forgiveness, again by keeping required payments as low as possible.
Let’s look at this coming tax season for example.
SAVE borrowers may suddenly be compelled to recertify their income. If you run ahead and file your 2024 tax return, 2024 income is now your only option when you got to recertify.
If you hold off and file an extension, you now have until October 15th to file your 2024 return.
And if your 2023 income was lower than 2024, that’s a good reason to hold out because you can recertify with your 2023 return (assuming they ask for it sometime before 10/15/25)!
It’s all about having the flexibility to choose amongst multiple tax years’ related income instead of being relegated to just one.
Granted sometimes it doesn’t matter that much. But it might matter a lot!
And filing an extension comes with NO penalties if you do it correctly.
You’ll have to make sure that you’ve paid-in a sufficient tax payment by April 15 even if you don’t officially file the return. But if you do that, there’s no harm in giving yourself more time to strategize.
(You will have to wait on getting a refund until you officially file, if that impacts you)
To be honest, I tend to recommend this to everybody because April 15 is just way too quick of a deadline in general.
1️⃣ exception
If you filed jointly with your spouse in 2023 and your joint income that year is significantly higher than your income alone in 2024 (and you don’t live in a community property state) then the advice is actually the opposite.
Particularly if there’s a chance that you will get a surprise income recertification requirement, like I mentioned about SAVE borrowers, you will want to get that 2024 tax return in as soon as you reasonably can. Honestly, I don’t think that this is something that’s going to happen before April. So don’t panic thinking this has to happen NOW. You cant even file your 2024 return yet if you wanted to. But this situation is definitely an exception to the rule I laid out above for most scenarios.
