I’m taking action to fix a broken prison system - but we have a mountain to climb. That's why I've ordered tough new checks, commissioned an investigation on systemic failures and begun overhauling archaic paper-based systems to get to grips with this unacceptable situation.

Mistaken prisoner releases are completely unacceptable. Years of cuts have hollowed out the system, and between 2010–24 just 500 new places were added. I will leave no stone unturned to fix this and keep the public safe.

William Smith is back in custody. The spike in mistaken releases is unacceptable. We're modernising prison systems - replacing paper with digital tools to cut errors. We're working with police to recapture Brahim Kaddour-Cherif.
