@YujiOkuno_OC Can you elaborate the nature of shoulder pain more? Transverse cervical artery is NOT one of the major 6 arteries when we do shoulder TAME, which are suparascapular, thoracoacromial, coracoid, circumflex scapular, ant/post. circumflex humeral aa.?

A woman suffering from severe shoulder stiffness underwent intra-arterial injection treatment targeting the transverse cervical artery. Her symptoms dramatically improved on the same day. Freed from years of discomfort, she has returned to playing golf.

@foggyleakybrain Sometimes the leakage is located anteriorly and not posteriorly, and can not be detected by surgical exploration. I've seen it in two patients.

@thjardi Yes, the patient did have blood patches, which they reported heavily reduced some symptoms, but only temporarily. Eventually 2 spinal levels were also explored in surgery, but sadly for this patient to no avail; as the puncture hole could not be localized

An anonymous story from a CSF Leaker (PDPH): "I had a badly performed botched lumbar puncture in August 2020 after presenting to emergency with a headache that I got at the gym. I am 95% sure I got my leak from the LP, not spontaneous SIH from the valsalva."

Late post but one of the true joys of my trip to the Kingdom 🇸🇦 was scrubbing and doing two GAEs with the wizard @MadkhaliDr - both cases done with only a 1.9 Fr access (discharged 15 min after case with 2 minute manual pressure) 😮 🦴 Thanks for teaching me your ways 🫡 #irad

@thjardi Do you ever combine with splenic arterial embo for reduction of flow/splenic size as well

In patients with hepatic encephalopathy and a large splenorenal shunt, not all livers can handle the extra flow after shunt embo. Splenomesenteric flow separation by splenic vein embolization can tackle this issue: the gut drains to the liver, the spleen drains to the shunt.

@SRajesh_IR I think you have to consider it in high HPVG and/or bad liver function. This patient had high MELD and severe neurological symptoms.

Today marks the 10 year anniversary of the discovery of the CSF-venous fistula. This has changed many patients’ lives. These fistulas cause spontaneous intracranial hypotension and are now being diagnosed all over the world. ⁦⁦⁦@spinalCSFleak#SIH #CSFleak #headache

@raimcsantos If thrombin fails: Direct puncture. Advance guidewire trough neck. Place angioseal.

@WBrinjikji @csfleakinfo Thank you for your answer. Finally we managed to join the contralateral internal epidural plexus trough the suboccipital plexus. Navigated all the way down to C7 then crossed the midline trough the anterior epidural plexus. Embolized with PHIL 25. Happy with the result !

Left C7-Th1 CSF-V fistula on dynamic myelography in a SIH patient. Currently planning transvenous embolization. How would you approach this? Probably too low for vertebral vein and too high for azygos system? Pre-procedural enhanced CT? #CSFVF #SIH
