@vgarg14 CV & non-CV mortality less frequent in mono group, so less major bleeding doesn't explain it all. Overall, this trial helps to support our guidelines recommending OAC alone in stable CAD + AF, but nice to have more trials showing the same. What are people at ESC saying?

AFIRE study: In Pts with AFib (CHADS2=2) & stable CAD (>1 year post-PCI/CABG if revascularized), rivaroxaban 15 mg/d alone non-inferior for MACE vs. rivaroxaban 15 mg/d + SAPT (mainly ASA). ⬆️ major bleeding & all-cause mortality with combination group. nejm.org/doi/full/10.10…

A question we often ask in the ED/ICU: Does this patient have increased ICP? Our @bmj_latest study found that traditional physical exam and CT signs had poor accuracy for elevated ICP. If suspected, consider treatment & transfer for invasive monitoring. bmj.com/content/366/bm…

@DanielJDrucker so today I think I downloaded yet another reason to avoid SGLT-2 inhibitors...BONE LOSS due to how the drug works...its very effective at inducing bone loss. that's okay, your blood sugar is "all better now" but you have osteoporosis & we just do not know why

Finishing two weeks on service today. About 10 patients with stroke. All over the age of 60. All had a normal transthoracic echo (TTE). I really struggle to understand why we do a TTE on these patients.
