@yuripridemd Totally agree, and I’m implementing it myself. My concern is the old issue of clopidogrel non-responders now being put on this drug. Although skeptical myself, if a pt develops an MI after switching, sb testing the pt to demonstrate polymorphism, then it’s hard to explain…

We knew clopidogrel was better a long time ago. And without a higher bleeding risk. Read CAPRIE. Was just really expensive at the time. Now it's my SAPT of choice for most pts when I'm stopping DAPT. Not novel.
CircInterventions@CircIntv

Clopidogrel is superior to aspirin for cardiac outcomes beyond 1-year up to 5-years. Results from the STOPDAPT-2 ACS and STOPDAPT-2 Total Cohort. Should it be a lifelong maintenance therapy? @HirotWatanabeMD @MNatsuaki @KoYamamoto7 ahajrnls.org/3PlxLhD

@drjohnm Those temporal trends are almost certainly driven by (1) ⬆️AF detection (e.g, smartwatches, etc) --> ⬆️recognition of less severe AF; and (2) ⬆️recognition/coding of risk factors which leads to "CHADS-VASc risk score inflation" c/w the original development studies.

I understand why the US and Israel is attacking Iran. It’s a loathsome regime that threatens both its own people and the world. What I can’t understand is why they didn’t do this before the IRGC slaughtered thousands of protestors?

Numerous studies show that pts who get LAAC are older and sicker than those in the trials This is unwise Spontaneous echo contrast is a marker of LA disease It also associates with a higher risk of stroke events and DRT. Of course it does jacc.org/doi/10.1016/j.…

For the last 47 years, Reza Pahlavi has barely been tested by the public or the press. He wants to lead a transition? He wants to take over a country in the middle of one of the most volatile and fragile geopolitical moments in its modern history? Then he shifts from being a symbolic figure to some Iranians to a political actor for all and political actors must get scrutinized. He is a politician now. He should be grilled — aggressively grilled. And he should be grilled by reporters and others who have expressed great skepticism about his ability to lead. His wife Yasmine Pahlavi should also be grilled by reporters — she’s not some quiet lamb in the background who isn’t involved in her husband’s life. I realize it’s painful for his supporters — many of whom have long viewed him as something close to untouchable — to see him asked difficult questions. (And let’s be honest: the questions so far haven’t even been that tough. They should be much tougher.) But welcome to the big leagues. Reza Pahlavi’s father, the late Shah, sat down with Mike Wallace on 60 Minutes and faced hard, adversarial questioning at the height of his power. At least three times, if I’m not mistaken. If you aspire to lead a national transition — especially one involving a country as complex and combustible as Iran — you don’t get to select only sympathetic interviewers. Iran is not a nostalgia project. It is a nation of 92-million people who are under immense internal and external pressure. If his supporters and advisors start whining about tough questions, it will only confirm what some of us already suspect: that the project cannot withstand scrutiny.

BBC Persian is the closest thing we have to an unbiased Persian news source and everyone hates it. To Islamists, it is a monarchy mouthpiece, to Monarchists, it's Ayatullah BBC.
Esfandyar Batmanghelidj@yarbatman

A notable exchange with @BBCKasraNaji. The content of Pahlavi’s answer was fine, but he still seems unsettled and defensive. He starts by cheaply insulting BBC Persian. Pahlavi is fully aware of the personal sacrifices that BBC Persian journalists have made. Why disrespect them?

Native PCI vs graft PCI after PROCTOR? RCT obviously doesn’t support native first, also no reflow is a lot lower for SVG PCI in contemporary trial vs perceived. Next question is to coil the SVG vs leave it open post native CTO PCI? Data free zone. @djc795 #CTO2026

Rajavi and Pahlavi have much different acceptance amongst the population and very different following! I don’t think that is a fair or an accurate comparison. I doubt these many people would have gone out had Rajavi put out a call! We saw the Berlin turn out last weekend. And please, stop with the “Pahlavi’s followers”. Everyone does it. Every person has extremist followers. Judge him on what his message is.

@MSharifpourMD @kshahrooz You listen to Pahlavi’s people’s slogans and you know what separatism is. You see how aggressive they behave facing non-monarchists. Besides: Just imagine how you would’ve reacted if Rajavi had invited! Would you still say it’s after all against IR?

This comment addresses people just like Mr. Shahrooz! The group that offers nothing and only complains. Feb 14 is not a referendum on Pahlavi. It’s a day to get together, regardless of our beliefs, to oppose IRI. He invited everyone, regardless of their background, as long as they are not separatists, to join. He didn’t say come out and vote for me! So, what value does Mr. Shahrooz’s declaration add here? Nothing! You don’t want to great, don’t go!

@MSharifpourMD @kshahrooz This very thoughtful comment is being disqualified by generalizing all Pahlavi-skeptics as bunch of losers. History will tell as for for the opportunities being missed by such a conduct

@expatsunleashed @tparsi Interesting how you talk about/to people who do have another opinion than you do. Even if this Video were fake , the way you criticize is telling me a lot for the time when you will be in power in Iran

I was waiting for you to post this video, it’s been aired by the Islamic Republic inside Iran multiple times already. A clear manufactured video to smear and slander the opposition. As if we needed any more proof that you’re the Islamic Republic mouth piece in the US. You shit-slithering lowlife.

Part of the reason a regime as unpopular and undemocratic as the Islamic Republic has managed to survive for 47 years is because of the undemocratic nature of the exiled opposition. In this clip, a representative of Reza Pahlavi visits an Iranian restaurant in Austria and demands that the flag of Monarchical Iran is put up on the walls. The restaurant worker explains that he is not the owner. Pahlavis representatives respond by saying that the flag must go up right away, otherwise they will "take care of the owner"...

You know, you can say you disagree with Pahlavi or you don’t like him or he is not qualified or you don’t believe in any form of monarchy and all are reasonable. But to call this a faceless revolution is at best misguided. As if you are intentionally trying to deny what is happening in Iran and what the people are chanting
