Also kudos to @rallamee very few senior investigators would have given the opportunity to their fellow to present the study at the AHA. She also had her other three Research fellows sitting in the same room as the Interview so they could get exposure to interviews by Press. This is what more senior investigators should be doing - investing in the next generation

Back to back PE last night.Didn’t get all the clot out but saw immediate drop of 10 mmhg in Mean PA pressures. Door to Thrombectomy in both cases <1 hour, oxygen requirements dropped from 4-5L/min to room air

In summary, the pharmacologic Rx for HFpEF starts with SGLT2i:
+/- loop diuretics
+/- MRA
+/- ARNI
+/- ARB (cannot take ARNI)

@michaelpflaher3 Recently encountered “friable leaflets” and had to abort after the first couple of grabs. Leaflet would start to breakdown with strands of tissue coming off soon after dropping the grippers. Patient wasn’t a surgical candidate. Started hemodialysis and he has done well.

Pt had to go to surgery…surgical input: friable leaflets “should have gone to surgery first”. Heart Team approach sometimes fails. Never know til you try.
