@jasonryanmd Were portable usg's not readily available either back then? I myself like minimalism when it comes to using resources. Think it also made physical exam and clinical skills very crucial back then something that has been diminishing

One of the most memorable cases I saw in training was in primary care clinic as a medical student. A young man came in early in the day with nausea and RLQ abdominal pain. Exam was benign but acute appendicitis was clearly in the differential. Lots of docs would turf this to the ER, but his PCP sent him home with instructions to come back at the end of the day. He came back, and the pain had not progressed, so he was sent home to come back again the next morning. The next day, the pain had improved and he now had diarrhea consistent with gastroenteritis. So, basically, this patient got outpatient serial abdominal exams for a possible acute abdomen. Appendicitis was ruled out with no labs, no CT scan. Just one doc using clinical judgement. Bad ass medicine.

I’m sorry if people have been having a go at you because of my tweet. Not at all the plan. I was very slightly drunk and already upset about something that had nothing to do with you. If it’s any comfort, I got it in the neck too. I’m a thin-skinned twat, apparently, even though it wasn’t my skin. I was sticking up for the writers who I adored. Obviously I shouldn’t have cited Bach/Kahlo/Moore - asking for trouble - and would have done better to go for the 10,000 blues songs written around the same 12 bar chord structure. I’ve listened to most of them and will keep doing so. Because we love what we love.

While it’s all been rather fun, I do hope @hughlaurie takes the time to read my @unherd article (I’m sure he can stretch to a subscription). Because while his witty riposte was genuinely amusing, one point I make in the piece is that it was shared with his 1.2 million followers on X. I have around 38,000. That creates something of an imbalance - particularly given that the responses to my original post were overwhelmingly warm-hearted and affectionate towards the show. The result has been some fairly horrific trolling. It turns out House fans are even more abusive than trans activists (and that’s saying something). I have enough experience of the media to take it on the chin, as the saying goes. But someone without my background might have found the experience far more distressing. Link below 👇

@jasonryanmd the problem is time management. Now they are giving 2 pages of h&p only to ask something that may or may not be related to the vignette. Another problem is too many ethics and QI questions that we don't really know how to prepare for beyond rotations and conventional resources.

Step exams are about 50% totally fair questions (stuff every doctor/student should know), 30% clever questions (fair but tricky), and 20% absolutely ridiculous exotic factoids. It's this 20% that drives most of the obsessive studying. You can't get those right without hours and hours of grinding and memorization. I often wonder what would happen if that 20% were removed from the exam. It would get easier and less stressful for sure.
