Mark enjoy that well deserved retirement. You were the first to let me do coronary angiography more then 25 years ago. I am forever grateful to you for that introduction to Intervention. A try legend in Cardiology Sir and you legacy shall continue in the Antipodes.

This is NIH with sessile thrombus which is now in the organising stages. I would have also used IVUS in this specific case to look at Adventitia and guide DCB treatment. Consider prolong DAPT and bring back at 9 months for repeat study.

@DrBIqbal @SyedYNaqvi1 @Ramku14 So Billy, I took off the table to discuss. But the consensus from all other Interventionists (apart from me) was PCI. Hence my tweet as I was surprised to be an outlier in the MDT as I thought the evidence in this case was clear to pursue OMT.

Lot of debate at the Cardiology MDT here in Sydney. 49 year old with CT coronary stenosis and angiography demonstrating lesion in LAD. RFR 0.92 after several measurements and FFR 0.80. Diagonal RFR 0.93, FFR 0.81. What do u do?
