The CWH "Connectors" from @AgeUKBANES, Bath Mind & other partners on another "trolley dash" through the wards of the RUH 🏥 Promoting all the benefits that the CWH can bring for those being discharged from hospital. In particular the "Home from Hospital" discharge bags.

We have partnered with Sara Dixon from @CWHBANES and Advanced Nurse Practitioner Nicholas Aplin, working towards improving frailty services in our community, increasing awareness about palliative and end of life care, and the importance of advanced care planning.

The Community Wellbeing Hub spent a great afternoon on Saturday at the RUH Community Day. Joined by Bath Mind in promoting all the services available for local residents. The CWH took 11,000+ referrals over the last year, nearly 25% were related to mental health and wellbeing.
