Remember that open conversations may have already taken place and have been documented. On admission check if there are Advance Care Planning documents . People may not be able to verbalise their wishes on admission but may have previously expressed them. #DyingMatters Awareness

Cheyne Stokes respiration at end of life- Thread Can be scary to witness for family, loved ones and health care professionals looking on. Understanding a little about what is happening may help alleviate some of that distress 🧵

One of my biggest regrets is that I did not know more than the first verse of Abide With Me as a first year doctor: I knew it only as a hymn traditionally sung, along with the National Anthem, before the FA Cup Final. How badly I let a patient down... ⬇️

Your annual reminder that the author of the words to “Abide With Me” was a Protestant clergyman called Henry Lyte. Regarded as a cold rigid and prickly man, he showed absolutely no poetic talent - at all - until September 1847, when he was dying of tuberculosis and in great pain Then, suddenly he sat down and wrote these words, in one burst. He died seven weeks later ****** Abide with me; fast falls the eventide; The darkness deepens; Lord, with me abide. When other helpers fail and comforts flee, Help of the helpless, O abide with me. Swift to its close ebbs out life’s little day; Earth’s joys grow dim, its glories pass away; Change and decay in all around I see; O Thou who changest not, abide with me. Come not in terrors, as the King of kings; But kind and good, with healing in Thy wings; Tears for all woes, a heart for every plea; Come, Friend of sinners, thus abide with me. I need Thy presence every passing hour; What but Thy grace can foil the tempter’s power? Who like Thyself my guide and stay can be? Through cloud and sunshine, Lord, abide with me. I fear no foe, with Thee at hand to bless; Ills have no weight, and tears no bitterness. Where is death’s sting? where, grave, thy victory? I triumph still, if Thou abide with me. Hold Thou Thy cross before my closing eyes; Shine through the gloom, and point me to the skies; Heaven’s morning breaks, and earth’s vain shadows flee; In life, in death, O Lord, abide with me

Naloxone in palliative care 🧵 Naloxone can save lives in people with opioid induced respiratory failure or apnoea. It’s use only indicated when there is a fall in respiratory rate with evidence of significant ventilatory failure

We are losing beds, team members and services that provide palliative and end of life care around the UK. @wesstreeting how do we maintain access to services? These are not people who can join a waiting list...

Could anyone recommend a good mental health therapist in Merseyside? It’d be good to talk to someone. On a waiting list of 20 weeks through the NHS😅

It’s important to identify people who are in the last year of life as early as possible. If we do not identify people who are dying, we lose the opportunity to provide the person with the best possible symptom management to achieve good quality care
