Latest 2026 Guidelines: Blood Pressure Targets [Very Important] 1. Preeclampsia and Eclampsia: Target systolic 140–150 and diastolic 90–100 mmHg. 2. Aortic Dissection: Target systolic less than 120 mmHg and heart rate less than 60 beats per minute. 3. Acute Ischemic Stroke with Thrombolysis: Target less than 185/110 before and less than 180/105 mmHg after treatment. 4. Acute Ischemic Stroke without Thrombolysis: Target less than 220/120 mmHg (permissive hypertension). 5. Intracerebral Hemorrhage: Target systolic 130–139 mmHg (avoid below 130). 6. Acute Pulmonary Edema: Target systolic less than 140 mmHg. 7. Hypertensive Encephalopathy: Reduce mean arterial pressure by 20% to 25% immediately.
