Lost a friendly bet on my Michigan Wolverines vs Minnesota Golden Gophers with @PennUrology colleague @beschur. I gave him a few points and that’s all he needed for me to be wearing these socks all day in the OR! (Didn’t keep them outside the scrubs all day thankfully.)

Seems to depend on one’s training. I trained at UPENN, and we tackle with 70 deg scopes and instruments. But I know this varies. Not everyone graduates residency comfortable using a 70 and doing frontals. Tho probably catching up as more and more programs have rhinologists teaching. I presume you also go with 70?

@JohnCraigNose Been using AEA flap with alloderm for a while. Last time had massive edema and long-term crusting on alloderm side. Eventually settled down but took months… did one recently with @AlanWorkmanMD and went flap only…. We shall see…

I used to use Biodesign until I saw a failure at 2 weeks with U/L AEA flap and the biodesign was GONE. Hence my move to Alloderm. But when biodesign works, i agree, it does seem to mucosalize faster than alloderm. But now that I get B/L mucosal coverage, the amount of IP graft exposure is nearly zero (tho does still feel good having it there, I know some feel it unnecessary)

@JohnCraigNose I think using a graft such as biodesign has made a massive difference. Not sure why, perhaps with another foundation for mucosalisation to occur.
