@julesnorton @davehartin @UHNM_NHS @Doti1208 Often the best people at taking on these challenging conversations are the heart failure nurses. The nursing teams do much of the care and often the first to recognise when patients are entering the more palliative phase of there disease.

@davehartin Absolutely agree, Surely a conversation in a clinic with someone who has mode information than I is better than a conversation in ED heart failure seem to have this right and talk about it early even though they may have years left @UHNM_NHS @Doti1208

@Doti1208 @julesnorton @davehartin @UHNM_NHS Too often in heart failure patients clinicians shy away from the difficult conversations meaning that plans are not in place and we have crisis management with patients inappropriately admitted to A&E. We all need to work together to improve care and improve EOL planning.
