UPDATE ON IMPORTANT NEW FINLAND STUDY | Professor Kaltiala has responded to criticism of the Finnish study, which found that mental health may get worse after young people undergo medical gender transition.
One critique of the study argues that psychiatric appointments are part and parcel of the gender-transition process in Finland and therefore may not point to serious psychiatric morbidity.
In their paper, Professor Kaltiala and her colleagues acknowledged the limitation that their study could not analyse in detail why these patients needed psychiatric services.
Asked about this issue, she told me that the healthcare register data used for the study included diagnoses and in-patient periods but not treatment nor information about a patient’s problems related to family life, romantic relationships or work.
However, Professor Kaltiala said the young people seen by specialised psychiatric services in her country would have “severe disorders”. These services were focused on “those really in need”, consistent with national criteria for health equity.
“In Finland, you do not get admitted to specialist-level services if you don’t have mental disorders at all or [have only] mild disorders and problems,” she said. “Primary-level services are available for needs related to milder disorders and psychosocial difficulties.”
She said the gender-referred youth in the new study were much more likely than matched controls to have a history of severe mental disorders, often dating back some years before they reached the gender clinic.
“Thus, many developed feelings of gender dysphoria in the context of severe disorders,” she said.
“Severe mental disorders during adolescence generally have potential to complicate identity development. It rather seems that feelings of gender dysphoria are sometimes secondary to severe mental disorders.”
If it were the other way around, and the mental disorders were secondary to the dysphoria, those disorders would be “expected to subside with medical gender reassignment”, according to the Dutch treatment protocol adopted internationally, she said.
But this did not happen in the new Finnish study.
Instead, the need for specialist psychiatric services—and therefore the scale of serious mental disorders—increased after medical transition, as the study by Professor Kaltiala and her colleagues reported.

🚨BREAKING: After an intervention by CAF, three colleges at the University of Cambridge have amended their #freespeech codes and reaffirmed protections under the Equality Act 2010 for students with gender-critical beliefs!
The development follows concerns raised by CAF about the treatment of gender-critical students, including Christ’s College undergraduate @theasewell05, a founder of the Cambridge University Society for Women (CUSW), the only openly single-sex society for women at Cambridge.
Sewell experienced sustained hostility after fellow students discovered that she had attended a talk by @HJoyceGender and read Trans: When Ideology Meets Reality. Within days, relations within her college deteriorated: she was confronted by fellow students, labelled a “bigot”, and ostracised.

In our letter to the Vice-Chancellor, we pointed out that gender-critical belief is a protected philosophical belief under the Equality Act and that conduct directed at Sewell and her CUSW colleagues on that basis may fall within the University’s own definitions of harassment and abusive behaviour.
We also noted that the University’s Code of Practice on Freedom of Speech, together with its harassment procedures, already prohibits conduct that interferes with lawful speech or creates an intimidating or hostile environment. On that basis, we asked what steps Christ’s, in particular, had taken to enforce those rules.
The letter also raised concerns about inconsistency between colleges. Accounts provided to CAF suggested that students holding gender-critical beliefs may receive markedly different levels of pastoral support depending on their college and the stance taken by individual tutors or senior staff.
Finally, we identified inaccuracies in several college free-speech codes, which incorrectly stated that the Office for Students free-speech complaints scheme was already operational, directing students to a route of escalation that does not yet exist.
In its response, the University confirmed that Christ’s, Newnham and Lucy Cavendish — the colleges attended by the CUSW’s founders — have now updated their codes and reaffirmed the rules governing freedom of expression and harassment.

📣🚨Karen Webb, an NHS specialist nurse with 42 years’ experience working with older people, faced losing her honorary Queen’s Nurse title after being reported for gender-critical posts on X.
The situation began during what should have been a standard team meeting. In the informal chat before proceedings began, Karen, in response to another colleague, expressed support for the Olympic Committee’s decision to exclude male athletes from women’s sporting categories. Within minutes, she received a private message from the Trust’s Head of Equality and Inclusion, informing her that she had “upset a lot of people”.
The following day, she was told that her “attitude” towards transgender individuals needed to be discussed.
Three months later, Karen was called to a formal meeting with senior figures at Cambridgeshire and Peterborough NHS Foundation Trust, where she was questioned about her views on trans people and her ability to provide them with compassionate care.
Believing the matter to have been resolved, Karen was then contacted by the Queen’s Institute of Community Nursing (QICN), which informed her that it had received a complaint about alleged “transphobia” and “bigotry”. She was instructed to delete her social media posts or risk losing her honorary title. The experience caused her significant and understandable distress.
At that point, Karen turned to the Free Speech Union for support.
The FSU wrote to QICN on her behalf, setting out her legally protected right to hold and express gender-critical beliefs under the Equality Act 2010. Shortly thereafter, QICN dropped its investigation and issued a full apology, acknowledging that there was “no case to answer” and that Karen had “done nothing wrong”.
Following a Subject Access Request (SAR), it emerged that the complaint had been made by a senior activist colleague, who had also disclosed information from a confidential internal process – despite that matter having been closed without any disciplinary action. This disclosure appeared to be a malicious attempt to damage Karen’s professional reputation and facilitate the removal of her honorary title.
A subsequent grievance investigation by the NHS Trust upheld Karen’s complaint, finding that confidential information had been improperly shared with QICN. The Trust accepted that Karen had a reasonable expectation that discussions within the internal process would remain private, and confirmed that this expectation had been breached.
The Trust has since stated that “appropriate action will be taken” to ensure lessons are learned and similar incidents do not occur again.
Although the Free Speech Union was ultimately able to protect Karen’s title and prevent disciplinary action, the experience has left a lasting impact.
Now retired, she reflects that the episode has cast a shadow over what should have been the culmination of a long and dedicated career in nursing. This sad episode resulted in Karen deciding it was time to retire.
We are proud to have supported Karen during this difficult time. Cases like hers are far from isolated – more than 40 per cent of the Free Speech Union’s casework now involves individuals facing repercussions for expressing gender-critical beliefs, which are protected under the Equality Act 2010.
